Ms KWAN Cheuk-yin, Jackie

Jackie (關卓妍) is a senior manager (Rehabilitation) of the Hong Kong Society for Rehabilitation (HKSR). She is a registered social worker, with a degree in social work and Master of Social Science from the University of Hong Kong in 1997 and 2004 respectively. She also obtained a post-graduate diploma in Mental Health from the Chinese University of Hong Kong in 2016 and she is also master trainer of Chronic Disease Self Management Program (CDSMP). Jackie has involved in the primary care services and developed self management programs for persons with diabetes, hypertension and common mental disorder. Currently, she is responsible for developing ICF-based (International Classification of Functioning, Disability and Health) stroke rehabilitation program and vocational rehabilitation services in the HKSR.
